Registered Nurse (RN) – Home Health Agency

Location:

Position Overview:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our dynamic home health team as an Independent Contractor. The ideal candidate will have excellent clinical skills, strong communication abilities, and a genuine desire to provide exceptional patient care in a home setting. This role offers flexibility and the opportunity to manage your own schedule while delivering top-notch healthcare services.


Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor and document patients’ progress and adjust care plans as needed.
  • Educate patients and their families on managing health conditions and promoting wellness.
  • Collaborate with interdisciplinary team members, including therapists, social workers, and physicians, to ensure coordinated care.
  • Provide wound care, IV therapy, and other specialized nursing services as required.
  • Ensure compliance with all healthcare regulations, standards, and agency policies.
  • Maintain accurate and timely documentation in electronic health records (EHR) systems.
  • Respond to patient inquiries and provide support to enhance patient satisfaction.


Qualifications:

  • Current and active Registered Nurse (RN) license in California.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 5 years of experience in acute and post-acute facility, home health, outpatient, or similar healthcare settings.
  • Strong clinical skills and ability to manage multiple patients effectively.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage your own schedule.
  • Reliable transportation and valid driver’s license.
  • CPR certification (or willingness to obtain upon hire).


Preferred Qualifications:

  • Experience with electronic health records (EHR) systems.
  • Specialized training or certifications (e.g., wound care, IV therapy).
  • Bilingual abilities are a plus.

  • This position is classified as a 1099 Independent Contractor role. Contractors are responsible for their own taxes, insurance, and benefits.

Licensed Vocational Nurse (LVN) – Home Health Agency

Position Overview:
We are seeking a dedicated Licensed Vocational Nurse (LVN) to join our dynamic home health team. In this role, you will deliver compassionate nursing care to patients in the comfort of their own homes. You will collaborate with RNs and other healthcare professionals to carry out individualized care plans, ensuring patients receive the highest quality of care and support.


Key Responsibilities:

               Patient Assessment & Care

  • Perform skilled treatment under the supervision of a Registered Nurse (RN).
  • Support the development of individualized care plans based on patient needs, physician orders, and agency protocols.


               Medication & Treatment Administration

  • Administer medications and other prescribed treatments accurately and in a timely manner.
  • Monitor patients for changes in condition or adverse reactions and report them to the supervising RN or physician.


               Wound & Specialty Care

  • Provide wound care, including dressing changes and specialized treatments as ordered.
  • Assist with catheter care, ostomy care, and other specialized procedures within the LVN scope of practice.


               Patient & Family Education

  • Educate patients and families on managing health conditions, medication regimens, and preventive care measures.
  • Offer guidance on lifestyle modifications and safety measures to promote overall well-being and prevent complications.


               Documentation & Compliance

  • Maintain accurate, up-to-date patient records in the agency’s electronic health record (EHR) system.
  • Ensure all services are delivered in compliance with regulatory guidelines, agency policies, and best practices.


              Collaboration & Communication

  • Work closely with the interdisciplinary team, including RNs, therapists, and social workers, to optimize patient outcomes.
  • Communicate effectively with physicians, patients, and families regarding care plans and changes in patient condition.


Qualifications:

  • Licensure: Active and unrestricted LVN license in California..
  • Education: Completion of an accredited LVN program.
  • Experience: At least 3 years of experience in a home health or similar healthcare setting preferred.
  • Certifications: Current CPR/BLS certification.
  • Skills: Strong clinical and assessment skills, with excellent attention to detail and ability to work independently.
  • Technology: Proficient in (or willing to learn) electronic health record (EHR) systems.
  • Transportation: Valid driver’s license, reliable vehicle, and proof of auto insurance.
  • Work Ethic: Commitment to patient-centered care, professionalism, and continuous learning.


Preferred Qualifications:

  • Experience with wound care or other specialized nursing skills.
  • Bilingual (English/Spanish or other languages) is an asset.
  • Familiarity with Medicare, Medicaid, and private insurance documentation requirements.
